Community Engagement Team

• Kristen Deppe’s view on Resource Map[ping is to create a new partnership while trying to consolidate and centralize building internal referrals for BR.

• Internal Referrals -Giving information, follow-up, Agency Advocates on patients behalf

• Central Location Referrals-Not having a lot of warm handoffs within agency. Trying to form a standardized process.

• Data Systems- BR does not have a single system for programs.

• Having flexibility within the Community Centers, Referral Centers and Day Centers has been a real challenge

• Apricot System was the previous data system used but posed a real challenge of connecting clients without multiple questions being answered by patients for referrals.

BakerRipley Team

• Would like to be able to manage BR internal referrals (no clear process in place right now, heavily rely on experienced staff)

• Need to have a centrolized database to gather up all internal resources

• Be able to allow people who know & are having on-going engagement with the clients to identify the referral (e.g. teachers, coaches)

• Used “Aprocot” system but not a big fun on the “recommended” referrals

• Question: Do you see BR more on the recieving or sending end?

• Question: We have several hundred employees who might be involved in this process, how would this work on the ground?

• You can have multiple champions for a program right?
