[HASI] Step 3: Complete the Social Needs Screening

Whether the patient agrees or refuses on doing the Social Needs Screening, the Screening Status will need to be updated. Navigate to the Social Needs Screening page from the Assessments dropdown menu from the navigation bar at the top of the page (H-21). From the Social Needs Screening page, click on the + Add new assessment button (H-22.1).

Step 3.1: No needs identified for the patient or the patient refuses the screening

If there were no needs identified for the patient upon completion of the Social Needs Screening Form, then mark the Screening status as Pt. was negative for all needs and will be screened again in 12 months (H-23.1). If the patient refuses to participate in the screening, skip to the Screening status selection in the form and select Pt. refused screening completely (H-23.1).

 

Step 3.2: Needs were identified for the patient

If there were needs identified for the patient, select one of the statuses below from the Screening status selection:

  • Pt. screened positive but declined assistance

  • Pt. screened positive and is open to supportive services

To add the needs found as follow-up items for the patient, make sure to check the Add Follow Up items care plan as action steps checkbox (H-23.2).

If the patient’s needs are in the HASI’s expertise, then proceed to Step 4. If not, proceed to Step 3.2.1.

HASI’S EXPERTISE:

  • Food

  • Housing

  • Transportation

  • Financial Support

Once the Social Needs Screening Form is completed, make sure to click on Save and Complete (H-23.3) to save the form on the patient’s record.

If you exit this page without clicking Save as Draft (H-23.1) or Save and Complete (H-23.2), your progress on the assessment will not be saved.
