What is UCCP?
Multi-Agency Access
The Unified Care Continuum Platform is built to support multi-agency access. This allows for streamlined referrals, data transfer, and outcomes measurements across an entire community. To protect the security of users and patients, different agencies and different roles within agencies have varying levels of access to patient information. When a user account is first created, a specific role is also assigned, and the account is granted the appropriate level of access. Roles can be changed by each organization’s System Administrator (or by PCIC).
Individual with access to multiple agency accounts
If you hold login credentials for more than one agency, you will select which agency’s account you would like to view right after you log on.
Multiple agencies contribute to same patient record
If you hold login credentials for one agency and your role allows you to view patient records from multiple agencies, you will be able to view your patients’ records including data for those patients collected from participating agencies.
