Assessments

The UCCP platform has been designed to support the implementation of an organization and/or program’s assessments. Because of this, the list of assessments available for a client will be based on the logged-in user’s organization or program.

Below are a list of assessments that is offered by PCIC, and may exist for a logged-in user’s organization or program:

  • Depression - PHQ9

  • EPDS

  • General Well-Being

  • Healthy Days

  • Quality of Life - DLA-20

  • Readiness to Change - URICA

  • SDoH Data Tracking

    • Neighborhood & Built Environment

    • Economic Stability

    • Food & Nutrition

    • Health & Healthcare

    • Social & Community Context

  • Working Alliance - WAI

An assessment may have automation with creating goals and action steps for a client.
