The sample assessment, the MBI - Human Services Survey, will walk through how to navigate and perform an assessment. Depending on the setup of the current logged-in user’s UCCP, the platform may or may not have this assessment available. Most assessments will direct the user to an index page of the assessment, where they can view all previous completed assessments done by the current logged-in user, and where they can perform a new assessment.

The assessment’s index page will show a table with the previously created assessments for the program (MA-2). Each assessment will have the following details readily visible:

  • Score – Raw score of the assessment.

  • Score Description – The relative score of the assessment (i.e. the achieved score out of the total possible)

  • Assessment Performed By – Name of the user who took the assessment.

Based on the current logged-in user’s role in the platform, the assessments shown here may just be the ones completed by the current logged-in user, or they may be all assessments completed by all users on the platform.

  • Assessment Completed On – Date the assessment was completed.

The grid can be ordered by column by clicking the desired column-header. The up-arrow denotes sorting by ascending order and the down-arrow denotes sorting by descending order.

Creating a New MBI Assessment

To create a new assessment, click Create a New MBI Assessment (MA-2.1). A pop-up window will appear with the questions for the assessment (MA-3). Click to select the desired response for each question. After clicking, the selected response should turn blue (MA-3.1). To deselect that answer, either click it again or select a different response; once unselected, the response will revert to gray, to signify its deselection. Users can also make comments on the assessment by using the Comments textbox at the bottom of the assessment window (MA-3.1).

Be sure to click Submit at the bottom of the window to ensure that the results are saved and recorded (MA-3.2). Responses can be changed and updated at a later time. If the window is closed before submitting the responses, the results will not be saved.

Viewing and Editing Previous Assessments

Click View Details under the Actions column for the desired assessment to open the assessment in a pop-up window with the previously recorded responses and comments (MA-2.2). Once opened, responses can be changed and comments can be added, as described above. Be sure to click Update at the bottom of the window to save the responses (MA-4.1).

Depending on the current logged-in user’s role on the system, and/or how their instance of UCCP is set up, the current logged-in user may only be able to edit their own responses, and not others. The current logged-in user may also not be able to edit any responses at all, and may only be able to view them.
