Submitting a Ticket

The UCCP Help Center allows our users to browse the different types of ticket requests that can be submitted or raised. Each ticket request follows the format below:

 

#1: The request type category (in this screenshot, this request is under Common Requests), which can be changed at any time in the request form.

#2: The request type (in this screenshot, this request is for Report a bug), which can be changed at any time in the request form.

Changing the request type could change the fields in the request form, depending on the request type.

#3: Select the UCCP program that you are raising a request for. For example, “Healthy Start” or “PCIC Care Coordination.”

#4: Provide a summary of your request. For example, “Opening the ‘View Clients’ page gives me an error message” (for a bug), or “New user - Jane Doe - Social Worker” (for a new user request).

 

#5: The fields to describe what you need support with. In the screenshot above:

  • A more detailed description of your request (Tell us more about what’s going on). Clicking on Show more… below the box will reveal helpful questions to answer in the description:

  • The URL link of the issue (What page is the issue on?)

  • The section of the page the issue is on (What section of the page is the issue on?)

  • The browser used (Which browser are you using?)

  • The device used (What kind of device are you using it on?)

 

#6: Any attachments you want to add to your request (Please attach screenshots and/or other files related to this issue).

NOTE:

If the screenshots or files contain PHI, please send these in a separate, secure email.

#7: Your email, or the email of the person you are submitting a request for. This email will receive all updates from the ticket.

If you want another person to get updates to this ticket, please respond to the confirmation email, or any email about the ticket, and say who you would like to share the ticket with, and their email address.

#8: Hitting Send will submit your ticket request, and hitting Cancel will cancel the submission.
