Phone Evaluation screen:

1. Login to calltracker, select evaluation link on any session for test, pcic provider.

   Evaluation template drop down available with Phone & Follow-up evaluation options

2. Select Phone Evaluation, enter all fields on HPI & Plan tab. Data should be saved on both clicking save button & switching tabs.

3. Click sign/submit or Output button. You can view the entered input data and draw signature by clicking the button.

Follow-up Evaluation screen: 

1. Select Follow-up evaluation from dropdown

2. Select all tabs on the left and enter all the fields. Data should be saved on both clicking save button & switching tabs.

3. Clicking sign/submit should pop-up a validation form if all the required fields are not entered. All the required fields can be entered and saved on validation form.

4. If all required fields are entered, you can view the entered input data.

Answering some questions refreshes the whole page, causes scroll up/down (likely revealing hidden fields?)—can be confusing at first

Editing the follow-up notes that have already been submitted allows you to submit and save with certain required fields blank

Are the checkboxes supposed to have a black background when empty/unchecked? Noticed this is not consistent throughout the form, some pages have white background to the check boxes, as expected
