Tracking the Participant’s Recreational Interests/Activities |
The platform now supports the ability to enter what the participant likes to do in their spare time. There are two questions related to this:
These questions are found in the Social & Community Context page of the SDoH Assessment. Navigate to the Social & Community Context page by expanding the Assessments menu on the navigation bar, expanding the SDoH Data Tracking sub-menu, and then selecting Social & Community Context (#1).The Recreational Interests/Activities contains two questions with two text-boxes to enter information in (#1). Once the text-boxes have been filled, along with any other data that is necessary for the participant in the Social & Community Context page, make sure to click the Save button (#2) to save this information to the participant’s record.

Track the Participant’s Relationship Factor with their Contacts |
The platform now supports the ability to track the following in the participant’s record:
This can be tracked in the Client’s Social Network section of the participant’s Demographics page. This contact can be marked as:
In the participant’s record, navigate to their Demographics page by clicking on the Demographics link on the navigation bar.Scroll down to the Client’s Social Network section in the participant’s Demographics page. Click on the + Add New button (#1) to add a new contact for the participant, and track the participant’s relationship factor with them. clicking on the View link under the Actions column (#2) will also allow the user to view or update an existing contact’s relationship factor.The Manage Contacts pop-up window will allow the user to create a new contact. Viewing or updating an existing contact will be a similar view. Scroll down to the Relationship Factor with the Contact drop-down selection (#1) to select the contact’s relationship factor with the participant.

Updates to Mailing Address Tracking |
In the participant’s record, navigate to their Demographics page by clicking on the Demographics link on the navigation bar.Scroll down to the Contact Information section of the Demographics page. Check the Is the Physical address same as the Mailing address? box (#1) if the participant’s physical address is the same as the mailing address. This will hide the Mailing Address fields (#2).

Allow for Exporting of Completed Visits & Tracking Grid |
The platform now allows for the Completed Visits & Events grid to be exported to an Excel spreadsheet (.XLS) or PDF format. Navigate to the Completed Visits & Events grid by expanding the Visits and Tracking menu on the navigation bar while in a participant’s record, then selecting View Completed Visits or Events (#1).Select the format to export the grid in the top-right corner (#1). The Excel icon button will export the file to .XLS format, while the Acrobat icon button will export the file to .PDF format.
