GAD-7 Assessment

 

 

While in a participant’s record, navigate to the GAD-7 Assessment page by expanding the Assessments drop-down menu on the navigation bar, and clicking on GAD-7.

The GAD-7 landing page will display a grid that will provide an overview of all past assessments:

  • Administered By, Administered On – Who administered the assessment for the participant, and the date and time that it was administered on.

  • Updated By, Updated On – Who last updated the assessment, and the date and time it was updated.

  • Completed By, Completed On – Who completed the assessment for the participant, and the date and time it was completed.

This page will allow the user to view all previously completed GAD-7 assessments, or administer a new one for the participant.

View a previously completed GAD-7 assessment by clicking on the View link under the Actions column (#1) for the desired assessment. Perform a new assessment for the participant by clicking on the + Add new assessment link (#2) .
The GAD-7 assessment. Be sure to click on the Save and Complete button if all answers have been filled out, or Save as Draft to return to the assessment later (#1).

Tracking Multiple Charges on Pre-Enrollment

The platform now supports the ability to track multiple charges for a participant in the Pre-Enrollment record.

 

The Charge Details grid when viewing a participant’s pre-enrollment record. Add a new charge for the participant by clicking on the + Add charge button (#1). View all previously added charges by clicking on the View Details link under the Actions column (#2).
The Charges pop-up window. Enter and select all details on the participant’s charge in this window. Once all fields have been filled out, be sure to click on the Save button (#1).

 

Tracking Financial Obligation for the Participant

The platform now supports the ability to track any financial obligations that the participant may have.

These individuals are picked from the participant’s Client’s Social Network section in their Demographics page.

To view/add a participant’s financial obligation(s), navigate to the Economic Stability page by expanding the Assessments menu on the navigation bar, then the SDoH Data Tracking menu, and selecting Economic Stability (#1).

The Financial Obligation grid in the Economic Stability page will provide an overview of information about the participant’s financial obligation(s):

  • Assists Person – The name of the contact that the participant is assisting.

  • Amount – The amount that the participant is assisting with.

  • Is temporary? – This box will be checked if this is temporary for the participant.

  • Edited By, Edited On – Who last edited this record, and the date that it was last edited on.

The Financial Obligation grid. Add a new financial obligation for the participant by clicking on the + Add Financial Obligation button (#1). View all previously added financial obligations by clicking on the View link under the Actions column (#2) for the desired record.
The Manage Financial Obligation pop-up window. Expand the Contact Name drop-down selection to choose one of the contacts in the participant’s record (This can be added from the Client’s Social Network section in their Demographics page). Enter the amount they are financially obligated to them. Check the box if this is temporary for the participant. Once all fields have been filled out, be sure to click on the Save button (#1).
