Attended multiple events to connect with community members:

• Participated in HFB 1^st Community Health Summit  

• Participated in a community event with FIT Houston w/ Fort Bend. 

• Worked with social work interns and increase community engaged 

• Increased office engagement  

• Volunteered for HFB  

Plan to increase partner community engagement in 2025 through partner events

• Make a video to utilize for community outreach to demo the referral product in use
