Client Summary

The Client Summary section gives quick highlights over the client’s progress and demographics.

This page may differ for the current logged-in user, depending on any customization required by the current logged-in user’s organization.

The following metrics are shown in this section (Figure CS-1):

  • Care Plan Completion – The percentage of completion of the client’s established care plan.

  • Delta Change in DLA-20 Score – The net change in DLA-20 score since the client began the program, calculated by dividing the initial score from the current score.

  • Number of Days in Intervention – The total number of days that has passed since the beginning of the intervention, or since the client began the program.

  • Number of Days of Intervention Remaining – The total number of days until the client’s expected graduation date from the program.

  • Time Spent (in hours) – The total number of accumulated hours spent with the client during a visit of any type (home visit, hospital, social agency, etc.).

Below the metrics offers more information about the client:

  • Enrolled On – The date of that the client was enrolled into the program.

  • Status – The enrollment status of the client.

  • Expected Graduation Date – The expected date of graduation from the program.

  • Insurance – Information for the client’s insurance, including the expected expiry date of coverage.

  • Progress – A brief summary of the metrics shown in the Client Progress section (percentage of care plan completed, costs saved, visits reduced and quality of life changed).

  • Client Notes – Any additional notes added by the provider or care coordinator.
