Provider-Level Dashboard Metrics and Graphs

Filtering Data by Provider

The Provider-Level Dashboard can be filtered to show all data from a selected provider’s cohort of clients. From the Provider dropdown, select the desired provider (PD-1.1). To view a dashboard that shows data for all of the clients for all providers for the current program/organization, select All from the Provider dropdown.

By default, the Provider-Level Dashboard shows data for All providers.

Filtering Data by Client

The default view of the Provider-Level Dashboard shows aggregated data for all clients who are served by all providers. To view the data for a single client, click on the client’s name (at the top) to view only the selected client’s data (PD-2.1). To deselect the client, simply click on the client’s name again (the yellow highlight will go away), and the aggregate provider data will be presented.

If the dashboard is filtered by a specific provider, then the list of available clients will be all clients whose care team they have been assigned to.

Provider-Level Dashboard Metrics

The Provider-Level Dashboard offers a variety of metrics to track a client’s progress through the program. Depending on the set up of the current logged-in user’s organization, the metrics listed below may or may not be included in their dashboard.

Client’s Visits and Costs

There are two graphs available that provide data on the client’s visits and costs:

  • Total visit count before and after intervention

  • Total cost before and after intervention

Data for both graphs can be filtered for 3, 6, or 12 months before and after intervention by selecting the appropriate button, as highlighted in (PD-3.1). When hovering over a colored section of the bar, the chart will display the exact cost or number of visits (PD-3.2).

Clients' Visits and Care Plan

There are three graphs available that provide data on the client’s visits and their care plan:

  • Count of Visits to Organizations

  • Time Spent by Visit Type

Data for both graphs can be filtered by when the visits occurred; for example, this month, yesterday, today, or the past three months. Expand the Visits occurred drop-down selection and selecting the desired time range (PD-4.1). When hovering over a bar on the graph, the chart will display the exact number for that organization (PD-4.1). This can also be done in the pie graph for the Time Spent By Visit Type.

Clients' Geolocation of Residence

The interactive map will display where clients for the program or organization are residing (PD-5). If the Provider-Level Dashboard is filtered for all clients, then the map will display data for all clients. If the Provider-Level Dashboard is filtered for a specific client, then the map will display where the specified client resides. Use the buttons in the upper left corner to move the map, or zoom in and out of the map (PD-5.1). Clicking on a marker will display the initials of the client that resides at that address (PD-5.2).

Clients' Overall Top 3s

The Overall Top 3s section (PD-6) provides a quick snapshot of the program and/or organization with the top 3 of the following metrics:

  • Successes

  • Challenges

  • Barriers

  • Resources Coordinated

If the Provider-Level Dashboard is filtered for all clients, this will show data for all clients. If the Provider-Level Dashboard is filtered for a specific client, then the metrics will display data for the specified client.

Clients' DLA-20 Analysis and Scores

An analysis of how the clients' quality of life has changed over time using the DLA-20 assessment are shown in the DLA-20 Analysis section. This section is divided into two: DLA-20 Questions, and DLA-20 Scores.

DLA-20 Questions

The DLA-20 Questions shows boxes for each category of questions in the assessment. The analysis of the DLA-20 questions can be filtered in multiple ways to gain a deeper understanding of these changes:

  • Compare Latest DLA-20 With (PD-7.1) – Select which answers to compare the most recently completed DLA-20 assessment to:

    • Overall for all DLA-20 assessments previously completed by all clients, or by the selected client.

    • Previous for the DLA-20 assessment that immediately precedes the most recent evaluation by all clients, or by the selected client.

  • Impact (PD-7.2) – Select the impact of the clients' or client’s DLA-20 answers:

    • Overall

    • Highest vs. Lowest

  • Rank (PD-7.3) – If Highest vs. Lowest is selected for Impact, then the Rank drop-down selection will be available. Select the rank of the highest against the lowest impact of the clients' or client’s DLA-20 answers. Depending on the rank chosen, there may be overlapping categories in 'Highest' and 'Lowest' impact visuals.

    • Choose from the Top 1 up until the Top 10.

    • All

Categories where the average score has increased for all clients or for the selected client will appear in green. Categories where the average score has decreased for all clients or for the selected client will appear in red.

PD-7: DLA-20 Analysis

 

DLA-20 Scores

The DLA-20 Scores shows a graph of the changes of each client’s DLA-20 score over time. The graph assigns each client a different color. Information on the client’s score and the date the score was given can be displayed by hovering over the client’s line on the graph (PD-8.1).
