Client Handout
The Client Handout can be printed out and given to enrolled clients to contextualize their progress using the similar visuals that the providers and care coordinators use to evaluate their progress within the program. It is a tool to increase accountability and transparency between clients and care coordinators.
Some information in the handout may not be available, or the handout will include more information other than what is listed below, depending on the set up of the current logged-in user’s organization.

Listed below are all data included in this report:
Client’s name
The date range of the handout
Client values and aspirations (Who/What is important to [Client], What would [Client] like to be doing) – For more information about the client’s values, please see Planning Care.
Care Plan Completion Percentage – The percentage of goals and action steps that has been completed by the client, to-date. The client’s goals and action steps can be updated in their Care Plan page. The client goals timeline can also be viewed in the Client Program (Intervention) Timeline.
Days with [Organization] – The number of days that the client has been with the current logged-in user’s organization.
To-Do List for You – The list of items in this section are items that are to be completed by the client. These items can be edited in the Care Plan page within the client’s record.
To-Do List for Your Care Team – The list of items in this section are items that are to be completed by the client’s care team. These items can be edited in the Care Plan page within the client’s record.
Your Medications – These are medications that are taken by the client. This can be viewed and edited within the Health & Healthcare page of the SDOH Data Tracking assessment, accessible from the client’s record. Read more about assessments: Assessments.
The following are actions that can be done to the Client Handout:
Export to the selected format – Expand the drop-down selection, and choose which format to export the report (Figure CH-1.1). Once selected, click on the Export link to save the report (Figure CH-1.1).

To refresh the handout, click on the refresh icon button (Figure CH-1.2).
Switch to print preview – Click on this button to view how the report will look once printed (Figure CH-1.3).
Print – Click on this button to print this handout (Figure CH-1.4). Be sure that if Protected Health Information (PHI) is printed, only authorized individuals will see the printed copy.
